Removing 1 of the 4 devices (PC was formated)

Hi Brave team,

I have been using brave on my 4 devices and today my PC for formatted and I can no longer verify it to my uphold wallet. Can someone help he remove my formatted computer so I can add the new device please? This post says we need to fill out a form but I cannot find it:
