How extend or add more items in initial screen like in image below?
Most frequently accessed or main sites. I want see ALL favorites bar in initial screen there is a lot of empty space to use.

@denisdamata short answer is you can’t. They leave all that space blank to fit any Sponsored Images (New Tab Page ads). Even if you have those disabled, they won’t let you use the area.
You may wonder, why? Well, if they let people who disable ads to use that space then they would incentivize that and lose money. Who knows if this won’t ever change, but for now it’s the intended design and they have not expressed any desire to change it any time soon.

Worth noting that while you can’t “expand” the widget that displays top-sites, you can add any number of “pages” that you can scroll through, using the buttons below the widget:
Brave could at least extend the top bookmarks bar.
No buttons it just show all favorites I need see it
It is unlikely that we will expand the widget in this way as @Saoiray stated initially. I was simply offering an alternative solution to the situation.
