How can this issue be reproduced?

Expected result: fill in user name and password like most other websites, but I am prompted to answer security questions every time i go to the website. after doing this once the browser is suppose to recognize the computer. it is not doing that and I have to go through this process every time i try to sign in (after shutting down the computer). until I shut it down, it will ‘remember’ me.
