So not sure what cause that, but when you have added sites to new tab sometimes these missing icons. When you click and open that website which not have icon then it comes back also sometimes icon are pixelated.
And you guys can think. it is because i did not put name under icon. No thats not cause that issue it happened aswell when there is name. Hopefully you can fix that, such annoying.
