How to remove devices from four lifetime limit?

I cannot verify any devices as all of four slots are full. I have replaced two computers and had to reinstall Windows on a third all before I realized that there was a limit. Now, I only have one device linked and would like fix that.

Previously, I saw a link in another thread for submitting a form to do so. Unfortunately, I cannot find it anymore. Can anyone help?
